Insurance
I accept all insurances as an out-of-network provider. Many insurance companies cover out-of-network mental health services, in full or in part, so call yours to find out your specific benefits. You can also use a Flexible Spending Account, offered by many employers, to cover any amount that was not covered by your health plan.
Please check your coverage carefully by asking the following questions:
  • Do I have mental health benefits?
  • How many sessions per calendar year does my plan cover?
  • How much does my plan cover for an out-of-network therapy session?
  • Do I need pre-authorization before beginning therapy?
  • What is my deductible and has it been met?
I will provide a monthly statement for you which you will submit to your insurance company. It is your responsibility to pay the fee, regardless of your insurance arrangements. You should be aware that insurance companies require a diagnosis which will then become a part of your medical record.
I offer sliding scale for people with limited financial resources.
 
Cancellation Policy
You are responsible for paying the full session fee for cancelled appointments unless I receive 24 hour advance notice. You are welcome to leave a cancellation message via my confidential voice mail (408) 882-5011 at any time.
